VerticalChange has the ability to generate a shareable link (URL) that can be sent to users or non-users to submit a form on the Program Level.

If Public Forms is turned on for your agency, you can turn on Public Forms for Program-Wide forms by doing the following:

  • Go to Forms

  • Select the Form that you would like to share

  • Click on Form Sharing

  • Click Share Form

  • Select Staff User (this is who will show as having created the Form)

  • Select Program - select the Program that the form should be associated with

    • Note: You will also need to be sure that the Form itself is associated with the Program

  • Click on “Use as Program-Wide?

  • Add a submission message that the user will see after submitting the form (Example: “Thank you for submitting your feedback!”

  • Save

  • This will now allow you to publicly share the form

How to share the form via a link (URL) after sharing has been turned on:

  • Go to Forms

  • Go to Form that you need to share

  • Click on Form Sharing

  • You will see a list of all of the Programs that have a shareable link (URL) associated with it

  • Find your Program and click on the “world” button

  • This will open the feedback form in a separate tab

  • Copy the URL

  • Send to user (via email or SMS)

Once the form is submitted, the form result will be viewable on the Program itself and in Report Builder under Programs.
