Creating Public Form - Form Builder

VerticalChange has the ability to generate a shareable link (URL) that can be sent to users or non-users to submit a form on the Contact Level.

If Public Forms is turned on for your agency, you can turn on Public Forms for Contact level by doing the following:

  • Go to Forms

  • Select the Form that you would like to share

  • Click on Form Sharing

  • Click Share Form

  • Select Staff User (this is who will show as having created the Form)

  • Select Program - select the Program that the form should be associated with

    • Note: You will also need to be sure that the Form itself is associated with the Program

  • Click on “Direct to Contact

  • Enter the Subject line you would like to be sent to the user

  • In the Email/Text Message field, enter the body of the message you would like to show when the user receives the emai/text

  • Select which Email and and Phone number fields you would like to use

    • Note: In order to use this feature, there must be an email address or phone number field in the Common Fields for Contacts.

  • Add a submission message that the user will see after submitting the form (Example: “Thank you for submitting your feedback!”
