Creating Public Form - Form Builder
VerticalChange has the ability to generate a shareable link (URL) that can be sent to users or non-users to submit a form on the Contact Level.
If Public Forms is turned on for your agency, you can turn on Public Forms for Contact level by doing the following:
Go to Forms
Select the Form that you would like to share
Click on Form Sharing
Click Share Form
Select Staff User (this is who will show as having created the Form)
Select Program - select the Program that the form should be associated with
Note: You will also need to be sure that the Form itself is associated with the Program
Click on “Direct to Contact”
Enter the Subject line you would like to be sent to the user
In the Email/Text Message field, enter the body of the message you would like to show when the user receives the emai/text
Select which Email and and Phone number fields you would like to use
Note: In order to use this feature, there must be an email address or phone number field in the Common Fields for Contacts.
Add a submission message that the user will see after submitting the form (Example: “Thank you for submitting your feedback!”
