Find the contact by typing in their name in the Jump To Contact search bar and clicking on the correct name
2. Once on the contact's profile, click on the Activities tab
3. Click on the hamburger settings button at the top right of the Activities grid, next to the Status column
4. Click on "Export all data as csv". An Excel file will begin downloading to your computer.
5. Open up the Excel file - you will see that each activity form is listed with its administered at date, staff member who administered the form, and the status
Export visible data as CSV - another option when you open the hamburger settings list is to "Export visible data as CSV". This button can be used when you only want to export form results that have been filtered.
Example: You type your name into the "Administered By" filter box to show only the form results that you have administered. Then click on the hamburger settings and click "Export visible data as CSV". This will export only the form results that are showing with the current filter.
Export selected data as CSV - if you would like to export only the form results that you have selected with the checkbox, first start by checking the forms you'd like, then select "Export selected data as CSV" in the hamburger settings.
You can repeat the same steps on the Assessments tab if you need a list of your contact's assessment form results.
