Adding to a Form
To add a Likert Scale Field to your form, simply click the Likert scale button under Available Fields section, or click and drag it into the desired location in your form.
Once it is added to your form, you can toggle open the settings and edit the field. For this field type, you can:
Change the Label.
Set it as a Required field (adds a red asterisk next to the Label).
Add, remove and change the label of each row of Statements. To add additional rows, click the + button next to the statement that you want to add a row below. To remove a row, click the - button next to the row you want to remove. You can also re-arrange the rows by clicking the up and down arrows next to each row.
Add, remove and change the label of each Column. To add additional columns, click the + button next to the column that you want a new column added to the right. To remove a column, click the - button next to the column you want to remove. You can also re-arrange the columns by clicking the up and down arrows next to each column.

Filling out a Form
When filling out a form for a contact or group with a Likert scale field, it will appear as below. You can select the radio button for each row, and type in any comments or notes if applicable.
If the field is required, an option must be selected for each row for it to be considered complete.
