How to complete guided forms on a contact's Dashboard

Guided Forms are a special version of program goals, and they link multiple forms together under one goal, walking you through filling each form out. 

In this example, we have a goal called "Complete Client Intake" that has an Intake Form Part 1 and Intake Form Part 2. When we look at the client's dashboard we see the goal title "Complete Client Intake" at the top, the goal description underneath the title, the deadline, and then the two Intake forms with a blue Start buttons to their right. 

Once you click on the first Start button, you will be taken for the first form in the set of guided forms.

Fill out the first form, then at the bottom you will have a set of options:

  1. Save & Exit to save this form result and exit out of the Guided Forms.

  2. Next and/or Previous to save your form result and take you to the next or previous form, depending on where in the list of forms you are.

  3. Save and Add Another, which allows you to save the current form result and fill out another result for the same form.  

  4. Cancel out of the form without saving.

While filling out your Guided Forms you can see your progress (what form you are currently on and what forms you have ahead and behind you) on the right side column

Once you have saved your form results, you can see them on the dashboard under the Form Name, along with a piece of information from the form results. 

Note: If you are seeing "No available data to display", this means that the piece of information that was set to display here has not been completed in the form result. To fix this, you can click on the "No available data to display" link, complete any missing fields, and save.

To the right of the forms, you will see either a Complete or Incomplete (in red) status, depending on form progress. If a form is complete, you have the option to Add a New form. If a form is incomplete, click the Resume button to be brought to the incomplete form. 

Each form result filled out for the goal will show under the form name so you can see how many results the client has for that form.

If you'd like to view or update data in a particular form result, click on the link on any of the form results to be taken directly to that form result.
