With the aim of simplifying data entry, starting in Fiscal Year 2017-2018, Intake Forms Part 1 and 2 will be combined into a single form. 

Rather than entering a separate Intake Form Part 2 on the primary parent, the questions for Intake Form Part 2 will be incorporated into Intake Form Part 1.

When entering an Intake Form Part 1 for an adult, you will see a new question: "Is this the Primary Parent/Adult Caregiver in this Family? (The person who completed the paper intake form)"

If you respond "Yes" this is question, the questions for Intake Form Part 2 will be shown at the bottom of the form. 

If you answer "No" (or if you are entering a child), the questions for Intake Form Part 2 will not show at the bottom of the form.
