Sending Public Forms - Direct to Individual Contacts

Sending Form Links

First, ensure that Public forms is set up the form you would like to send. Click here to learn how to set up Public Forms on the Contact Level

Once you have Form Sharing turned on for your forms and programs, you can now send a link directly to a contact from their contact profile.

  1. Go to the Contact

  2. Click on the "Outside Forms" tab

  3. Click Select Sharing and select the form that you'd like to send

  4. Select the method (email or text)

  5. If there is not an email address or phone number in the Common Fields for the contact, the link will not send

  6. Once the user submits the form, they will not be able to complete the form again

  7. If you need to resend the form, you are able to do so from the Outside Forms tab. You will see Resend button on the right hand side.

  8. You can also view the status of the links you've shared in the Outside Forms tab

  9. Once a contact has submitted the form, you will see a link to the form in this grid. The form result will also show on the Activities/Assessments tab and in Report Builder

Access to sending Public Form links is driven by permission sets. You will need to enable the following permissions in order for a user to be able to use this feature:
